How well do those stove-top milk steamers work?  Is there any particular brand that is better/worse than the rest of the pack?

You can always find a steam espresso maker ("steam toy") at a thrift store and use that for steaming milk.  They make a lot of steam and would be a good inexpensive way to do that.
