I think you're going to have to play with it a little until you manage to get a vortex going. You could always do a test run with some water, before wasting milk. Once you figure out what angles you need and your steam tip position to get a good roll or vortex, you can then switch to milk and see if you can get a good texture...might want to buy a gallon or two just to practice and hone in your technique (or maybe it's still a bit early in your process for that?)

Just an update:
I had to give up on the tooth picks. I wasn't able to get it to work, maybe the wrong shape of tooth picks? It was leaking.
But my local supplyer was able to sell me a new one tip hole tip for approx. 10 dollars, so I figured that was worth a try. And that has been a huge success. After a day of getting used to it I have been getting far better results than with the 3 hole tip. There's way way more power and swirling than with three holes. I can truly recommend doing this. Especially since I could just change the tip and not the whole arm.
