Looks like a Rosito Bisani RR45A which is just a re-badged Gino Rossi RR45A. As it is one of the most common grinders used in coffee shops; parts are reasonably priced and easy to find. The 'A' and the end of the model number indicates automatic (the coiled cable on the top of the doser is the clue) which turns on the grinder whenever the fill in the doser is low. You'll want to disable this 'feature' for home use.
These show up all the time on Ebay (6 right now) and tend to go for considerably less than the Mazer Super Jolly ($150 would be a good price for a fully functioning unit).
